I've experienced all of these at point or another. Job I left last year was especially bad with number 5. Small company, owner/president. Was remarried and screwed over his daughter from his first marriage (my boss) by replacing her with a much younger daughter (25 years) from his second marriage. He had also brought in his son from second marriage, who was totally incompetent. If they will target their own children they will have no qualms about going after anybody else. I was number three on the list, several others after me were also on the list.
